Is there anyway to escalate it to the help team or do they just pick it up eventually?
They'll just pick it up...
How would I raise a speed fault?
You could try the fault bot here https://www.plus.net/help/report-a-problem/ but TBH it often doesnt work on broadband only accounts. Otherwise its call CSC and report your speed below the handback threshold . They're likely to request a check at the test socket though...
Would the fact the Plusnet router has only ever been in bridge mode cause an issue?

I guess this confirms that I'm definitely on an 80/20 line with the speed going above 40?
It does. I'd be inclined to leave it as is for a day or two. If there's no improvemnt, then try removing the master socket faceplate and trying the test socket. It has been known for the Mk4 faceplate connections to deteriorate ( you might even just try making sure its clipped on securely! )

Be aware that there are two different filtered faceplates. The latest clip on type that you have linked and the earlier type that is held on with two screws. They are not interchangeable.

If the router has only ever operated in bridge mode, would that preclude TR069 management and checks for firmware updates?
In another browser tab, login into the Plusnet user portal BEFORE clicking the fault & ticket links

Unless the router makes the PPPoE connection its not able to access the TR069 management platform

Just an update on the speed, this morning I changed the rj11 cable to a new one (the one I was using was years old as the Plusnet router didn't come with one) and plugged it into the master test socket (I've got no home phone so the extension sockets aren't in use).
My speed has gone from 45mbps to 50mbps which is a decent bump, I'll monitor over the next few days to see if there are any other changes

The speed increase looks promising. It would be interesting to log back in to the Technical Log as per post #3 and see whether there has been a significant increase in the noise margin. If so, the DLM should reduce this and hence increase your speed over the coming days. With a good line the noise margin will reduce to 3dB.

Extension wiring - especially (incorrect) star wired - does not have to be in use to cause havoc. It is more likely that the elimination of the telephone extension wiring than the 'new' DSL cable is the 'cause' of the improved sync rate.
Unnecessary extension wires are best removed.

data rate 7.781/54.112
maximum data rate 7.781/55.293
noise margin 5.9/4.2
line attenuation 11.3/18.4
signal attenuation 11.3/24.8
My maximum download speed was 45mbps
My router stats are now
data rate 7.888/55.46
maximum data rate 7.888/56.355
noise margin 6.0/4.1
line attenuation 11.1/18.3
signal attenuation 11.1/24.6.
My maximum download speed is now 49mbps.
I'll monitor again in 24-48 hrs
