Reward card not received

Hi, I have been a Plusnet customer since the start of April and was supposed to receive an £80 reward card.  Thus far i haven't had any emails through from anyone about claiming this. Do i need to do something to start this process?

I am sorry you have yet to receive the reward card. I have checked your details and can see you have been liasing with my colleague therefore I will be closing the ticket.
