not really certain which forum you are referring to live or beta, but for instance Poppy has the blank one on live, Orbrey has a picture

Poppy's avatar has always appeared like that (the only difference being the outline on this skin)
The reason some work on beta and some don't is that the uploaded ones haven't been copied across to the beta site. (ones linked to hosting outside the CS work fine)
The reason Chilly's avatar isn't working on live is because he hosts it on his own server at home, which throws the occasional wobbly.
