I would like to find out whether I am on a monthly rolling contract or a 12/24 month contract. Also, how do I find out whether any fees would become applicable upon cancellation?

Have a look here: https://portal.plus.net/my.html?s=0&action=account_summary ; (you'll need to log in). In my case because I am on contract there is an entry that says £xxx (upon cancellation) which I presume would not be there if I were on a rolling contract. There is also a broadband cessation charge of £30 shown but that only applies if I don't move to another broadband provider but cease the service completely.

That is a bit more difficult
I suggest going here http://www.plus.net/info2/legal/price_guide.html and looking for the Early Termination charges - you should then be able to work out how many months you have left.
