@Ninko looks about right to me, as long as there's no VLAN ID set somewhere else
Username should be your Plusnet account name suffixed with @plusdsl.net and the password is the one you use to access the member portal at www.plus.net/login

I'm wonder about the NAT type setting, is Symmetric correct?

If you don't know what VLAN ID is then you've almost certainly not set it. I only asked because if you read the plus net help for setting up your own router , it mentions using a vlan ID for Fibre. In the case of full fibre , the ONT handles the plan requirement and so the router should not set it.
As for NAT type , I wouldn't worry, most routers don't even have the ability to change it!
