I'm digging into this further and I don't believe we did cancel your direct debit instruction.
The note on your account that Emily saw was likely an automated ticket generated when your new DD went live, advising that the previous DD was cancelled on our systems.
I think that the cause of the problem might be the duplicate DD setup at your banks side.
Can you click cancel on the instruction ending with ref -3 on your banks interface?
Once that's done, please do let us know how you get on when your billing date comes around.
