What exactly is the problem you are having? Any error messages, things not happening etc?
A little more detail about what exactly you are trying to do will help us to help you, along with a URL to the form page in question.
Is this an HTML base form, a Frontpage based form or a PHP based form. Where are you uploading it to. If using FrontPage you need to publish it to fp.<username>.plus.com from within Frontpage, not via FTP.
