Will the new full fibre cable follow the same route, or are they likely to want to enter via the ground floor?
The thought of running an ethernet cable around the house is worrying me. It's possible, but time consuming and messy.

As stated the fibre has to come down to ground level to be joined (fused) to the house connection. The engineer may be more than happy to take the cable back up and install the ONT upstairs.
Unfortunately you just don't know until the day of install. You do speak to them to agree the work.
If the ONT has to go downstairs for whatever reason consider running an external grade ethernet cable out and up the wall to back in at the upstairs room.

Whilst the preferred / generic mounting position for the CSP is might be on an outside wall just above ground level, that is not the only option.
My fibre cable enters the house under the eaves and my CSP is mounted in a utility cupboard, albeit on the ground floor, although I suspect I could quite easily have persuaded the OR installer to mount it on the upstairs wall near to where the ONT is. And the OR tech was quite happy to oblige when I asked for the 30 metre* internal kit to be used so I have approximately 25 metres of the internal fibre coiled up under the floorboards below the ONT in case I wish to move it in the future.
ISTR reading at least one account on here fairly recently of a CSP being installed in a loft, so it might be more accurate to say the CSP must be installed just above floor level, although yet again, mine deviates slightly from that rule being 1.39 m above floor level at its centre.
* 30 metres internal was an option at the time of my install, possibly not available nowadays.
