This is now resolved. It was a issue within the BT node which we started using a couple of months ago.  It become evident when we start sending over a certain amount of traffic.
Any other packet loss issue will be a different problem.  Haltdef:  Still seeing it?

Still some packet loss, nowhere near as bad and doesn't seem to be affecting throughput now. Look normal?

Are you running some kind of service/program on one of your network devices?

Quite a lot, I've tweaked my QoS settings a little, DLM has been playing with my line a lot recently - actual upload speed had dropped a bit so I've told the router about it, see if that helps.

Can you start a new thread so we can leave this one to die detailing what you've got?
I think we might need to return everything to a refresh start, removing your router customisations so we can see if it's actually your line, or your tweaks.
