Have not received form to claim £50 cashback

Hi, I took out a package for free weekend calls and unlimited broadband through Uswitch on 1/8/2015. When I took it out, there was a £50 cash back offer but I haven't received the form I'm required to complete in order to claim the cash back. Would it be possible for some one to send it to me? Many thanks!

Apologies first of all that we haven't replied to your post yet.
I've just chased this up for you and can see that due to the time scales involved you should receive the redemption email next week.
