I recently reported a slow broadband speed. The response was excellent and within a few minutes the speed was restored. It was good to talk to a technician who knew what he was doing. Later I received a text message asking for feedback. I' ve spent ages trying to find out how to do this!!! This posting is the nearest I can get, but is thre not a formal customer satisfaction form.
The text message should have been accompanied by an email message sent to your normal email address. The latter should contain a link to a survey site for you to complete an online questionnaire form. If you can't find the email there should be a copy in a closed ticket. You can view tickets by going to https://www.plus.net/wizard/?p=search. Click the Go button to see those closed in the last 7 days (select a longer period to go back further).
