Failed installations - how many customers are you losing?

We've seen two examples on these forums today where OpenRetch have let you down with installations and the new user has lost confidence in Plusnet and cancelled.
I'm sure we're only seeing a fraction of them in here -so how many users have you lost recently as a result of OpenRetch's incompetence?

Can you point me to those examples? I'm only personally aware of one that appeared today an Openreach weren't the cause of the issue.
We'd never be able to give you precise figures about customer churn and the reasons reported for it but this is an issue we're obviously aware of and we're looking into what we can do to work with our suppliers and look at some of our own processes too to help avoid such problems impacting on our customers in the future.
