Connection settings for TD-W8968

Hi, I have had problems with my router after accidentally deleting some settings earlier this evening. I have a static IP and am registered via free-online. I have run a factory reset so it's all fresh. Until I broke it this evening it's run for years without issue.

Can anyone advise the connection settings required?
