No reward card

Hi,

 

I was hoping somebody could resolve an issue for me regarding my broadband subscription. I've just spent almost 20 minutes on hold on the phone so don't fancy having to call again. It's been over a month since signing up to plusnet and my broadband has been active since 28th May however I have yet to receive a £70 reward card or email. I'd just like to get this resolved as soon as please
