When does the 12 month contract start and end?
1. 365 days from the day I paid?
2. 365 days from the day the the service start?
3. Last day of the 12th month?

It starts on the day your first service 'goes live' and ends at the end of the twelve-month period, i.e. if your broadband goes live on 15th May and your phone on the 16th May, your contract is considered to have started on the 15th (they'll give you a one-day credit on your phone bill), and the last day of your contract will be 14th May next year.
This is based on personal experience of what the T&Cs were some two and a half years ago when I joined, but I doubt they've changed in this regard.
