I was looking at my online banking earlier and noticed 2 DD mandates had been setup for Plusnet. I never setup a second one, my account details haven't changed and i've never had any problems with the current one. Can anyone please give me a reason why this has happened?

Were you by any chance with Alliance and Leicester? They are moving all customers direct debits over to the Santander sort codes following the takeover a couple of years ago.
