Tried 3 times to set up account by phone, does not go through by payment taken

Hi,

 

I've tried now three times to set up my account over the phone with [CSA Removed] (who has been great). However, each time I get to the billing stage, the account does not go through, but payment has now been taken three times with two different cards, neither of which are blocked and have plenty of money on them. I don't really understand how this is possible. The bank assures me nothing is wrong, so it must be on the Plusnet side of things.

 

Any help on this would be great.
