Can I submit a cancellation request here or does it have to be done over the phone? Thanks

Be careful what you wish for! I asked for a MAC, Plusnet tried to terminate my service immediately. I migrated, Plusnet charged me a cessation fee that isn't supposed to be payable if you migrate. Plusnet cancelled my DD and then told me I hadn't paid the final bill. I paid the bill by debit card. Plusnet apologised for the poor customer experience. Today I get an email telling to pay an unknown amount outstanding in 14 days or it will be sent to an external agency and I will be charged for this. I can't even log into my account, it has now been cancelled too. It is easier to get a divorce than leave Plusnet.

I had many good years with plusnet, but your handling of my switch to fibre left a bitter taste in my mouth and your latest price increase was the final straw. I won't be coming back.
