Quidco cashback tracked, confirmed, then declined!

I signed up to Plusnet on 1.11 through Quidco, for 100 pounds cashback (broadband + landline). The cashback was immediately tracking and shortly confirmed, but now it is showing up as declined!
I am trying to raise a claim through Quidco but am asked for an account number - am unable to find this anywhere...
Could you please look into this? I have been active for a while already!
