Need to cancel mother's contract

Hi.

I manage my mother's contract with you. It's for line rental, phone and broadband. She has now moved into a care home, and her house is for sale; there's no possibility of her returning to the house. The contract with you is still withn the contract period. Please could you let me know next steps to cancel the contract?
