Giving Notice

Hello,
I'm moving out of my house shortly, back to my parents (they already have broadband and are in a contract).
How do I go about ending my service, and how much notice do you need? I'm not in a contract.

Also, does the 12-months (for the £40 router charge) start when you place your order or when Openreach actually show up?
I joined Plusnet in July but service didn't start until September.
