Double-Billed & Cashback No Communication

Good morning.

Hopefully someone from Plusnet will be able to advise on the following:

Issue 1: I signed up to Plusnet on April 2019 and despite checking my emails and calling, I have not received any communication, either at sign up or since, about the £40 cashback that was an element of the contract that I signed up to, I should be grateful if someone would be able to advise when/how this will be actioned.

 I can confirm that I have checked my email account, including spam, and no email has been received on this subject.

Issue 2: I checked through my bills and noticed I had been double - charged a month, via direct- debit in December on both the 3rd and 30th 2019. 

Please could the above two issues be rectified?
