Direct Debit Offline Form

Hi,

 

I have recently joined a new company and they have offered to pay for my internet. I would like to have a form I can give them so they can take care of the direct debit. Please can you let me know how we can do this as if I log on they need my details and vice versa?

 

Traditionally companies used to offer paper direct debits forms. Is this possible please as it is causing my colleagues and I a headache?
