Broadband Genie - reward card

When I joined Plusnet I signed up through Broadband Genie to an offer including a reward card - I haven't received any emails about the reward card and now that my account is fully set up and running I had almost forgotten about it. How do I go about claiming my reward card?

From what I can see the offer you signed up on isn't showing as to have included cashback. So I can investigate further can you drop me a private message by going Here with the cashback amount you were offered?
