£75 Reward card — email not received

I signed up for the broadband deal with a £75 Reward card. 

My broadband account activated a month ago but I haven't received the email about the Reward card yet — could you send it please?

(This is obviously a question for Plusnet, not the forum! Phone lines are busy...)
