Roaming data charges despite phone settings

My latest bill includes some (very small, but non-zero) charges for roaming data, during a time when I was abroad, and had mobile data turned off on my phone. The itemized bill shows 0MB, and charges of a few pence on each of several days. How can this happen?

-your full address including postcode

Have just had this exact problem. Currently abroad and mobile data been off, small roaming charge being applied each day. Had this once before but was fobbed off, but now have a completely different phone too. Any ideas?
