Proof of Usage - Insurance Claim

Hi There, 

My phone was stolen last weekend. I am filling out an insurance claim and they require a document outlining proof of usage with an IMEI number. 

I called customer service who suggest I print out my bill from the website. Unfortunately this will not suffice for the insurance company. 

You could please help in securing proof of usage that includes my name and the IMEI number?
