Lost phone/sim

Hi,

 

I lost my phone yesterday and need a replacement sim (same number if possible?) but the only way to contact is phone and I dont have a phone.

 

Little bit stuck on what to do

Characters 5&13 from "your memorable place"

The last 2 digits from your bank account number

The last 2 from the sort code
