Double billing, warning emails despite not using the service

A DD has been taken from my account for double the usual amount can you provide an explanation please?

Also I was told I would get one months rembursment due to the delays setting up the services during the first month as it was taking time to setup?
I srill cant get it to work and I accept all other 6.50 monthly charges except this latest double bill.
Thirdly why do i keep getting emails stating I have used all my monthly allowance and have been prevented from using any mobile telephony?
