2GB Mates Rates

I am an existing Plusnet Mobile customer and signed up to Plusnet Fibre Broadband which has been live from the middle of December. Could I please get the extra 2GB mates rates added to my sim.

So I can get that arranged for you can you PM me the following?
- Your mobile number or account number
- Your full address and postcode
- Your full name and if you're the account holder?
- Your date of birth
- The last 2 digits of your sort code and the last 2 digits of your account number used to pay the bill each month
