I thought GPs were essentially self employed anyway, and get paid by the number of patients on their books.  Paying them another £20 would simply give them an even higher rate of pay (I believe the average GP earns around £150,000 a year (plus of course their share of the company's profits))
We have to import out of hours services from the continent, because our GPs are no longer obliged to work outside normal hours, and if they do, they get paid (a lot) extra, but at £150K p.a., they don't need to.
