Not in the same league, but I once heard a Radio 2 presenter do a dedication for someone in Bury Street, Edmunds.
And when Radio 2 still had a MW frequency, which sometimes split from FM, a continuity announcer said "...on medium wive, meanwhale..." and very professionally continued without missing a beat - or maybe she hadn't even realised.
