With ADSL there is no charge for switching between ISPs who both utilise BT Wholesale infrastructure.
Looking Fibre services from various ISPs there is almost always a set-up charge. Do these charges apply to customers who already have a Fibre service when switching to a Fibre service with another ISP (assuming that no "free" router is required)?

Yes they apply to migrations too.

However this charge is varied by all ISPs when they create new customer offers so the headline cost figure can be cut lower but the one-off install costs can be pushed up. Or not sometimes.
Basically this one-off cost has no relationship to the true cost of getting your service live. The amount you pay is normally a lot less than the wholesale cost of setting up. And the rest is recovered over the contract period from your other charges.

And of course you can always offset the setup fees with cash back and the like.
I recently migrated from Sky to BT, and I was still £200 up even after taking into account the £60 setup fee BT charged.
