What does 'heavy' mean at the end of a sentance?

It normally means that the pilot is calling from a big plane

Looking at some of the other videos on Nozzer's link I came across this.  It's the last flight of Concorde from JFK in October 2003.  Very emotional. What a beautiful plane Cool
