FTTC Service Processes

Dear Ben, Dave and Craig,
it would be good if you could outline to the people in this community the steps involved at a detail level in moving from ADSL to FTTC showing who does what between the exchange, the cabinet and the house for the physical connections and then the network connections. The FAQ is great but lacks this detail.
Having fallen foul of it myself, it would also be good to indicate what happens during the trial if a fault occurs, what are the likely resolution times and whether you have service levels agreed with openreach and wholsale at BT.? If so, what the agreed levels of service are so we can know what to expect for different fault types.
Putting it out here would be an open and transparent way of dealing with the bedding-in issues and would not raise expectations too high.

I don't have the detail of the provisioning process, but to respond to other questions:

A fault should be raised via http://faults.plus.net as normal.

These are unknown, one of the purposes of us running a trial is to identify faults processes and expected lead times.

Just as with ADSL, we do not have SLA's with our suppliers.
