Unable to connect following modem fault (FTTC)

Following intermittent connection problems which became constant an Openreach engineer replaced my modem.  He was able to get DSL sync with 40M upstream and 10 downstream.  However I am still umable to connect.
[mremoved] - CSC - Team Managers then sent me test results which showed a stable connection for days despite having no connection with multiple power offs of modems and routers and line tests by the Openreach engineer.  
I have been unable to connect even with the bt_test@startup_domain login or bypassing router.  Looking at the various guides an Authentification problem looks possible.
Anyone have any ideas what I should do?
