Sporadic Packetloss makes streaming hard.

I've been a customer for awhile now, we've had issues with our line, but through it all I still seem to get issues with high ping and occasional packetloss/stream drops.
It affects ingoing and outgoing streams, youtube will start buffering and not recover, netflix streams will drop out and I get lots of lost frames on streams, this has been going on for well over a year as the last time I use the thinkbroadband graphs I was seeing the same thing but as usual PlusNet said they couldn't do anything about it.

Here's some Twitch Inspector graphs, you can see all the drops: I used to sync at 75/18Mbs, this should be MORE than enough for a 3Mbits stream. (Yes I have tried other ingest servers, if anything that makes things worse)

You might notice a couple of offline moments in the TB graph, I did a couple of quiet line tests, first with just waiting after the dialtone and the second using the 17070 Quiet line test. There's certainly some very faint noise on the line. And before any admins ask, it was using a BT hardline phone straight into the test socket, with the phone on mute.
The extension was installed by a BT engineer, the original socket is completely bypassed. The engineer said that as far as we're concerned, this is the new master socket.
EDIT: I also rebooted my router at 11:25pm
May be unrelated, but when I was doing the first test (And I keep noticing this), when the dial tone goes out, exactly 30sec later there's a massive *CRACK* then some hissing and clicks then another massive *CRACK*.
Lastly I found the twitch bandwidth tester graph isn't very good for showing short dropouts. I'll use a test account and a live stream to test again, but I am noticing the dropped frames, even with WiFi off and nothing else connected.

Just had a strange period of downtime at 2:55am (Mon 28th).

(Seems my router needs to learn about BST)
Connection came up a few minutes later but there was an authentication failure o_O

This seems to happen occasionally in the midnight hours, was BT fudging my line or did PN tweak something?
