Packet Loss?

The graph below tells the story - My line has recently been experiencing sustained packet loss.
Are there any explanations or solutions?

Sorry for the slowness in replying...
Here's a live BQM:

Packet loss seems to have gone after a PPPoE session reconnect & modem reboot. No idea why this could be though Cheesy

Hmm, related to the congestion issue perhaps? A gateway hop has obviously resolved it. Logging into the Router and clicking Disconnect, waiting 30secs and the clicking Connect is the best way of achieving a Gateway hop..
I wouldn't resort to rebooting the modem willy-nilly either.

Modem had a 10+ days uptime before the reboot. I usually try hard to make sure DLM doesn't kick in.

Rebooting the Modem on Fibre is generally a fruitless exercise unless there is a specific reason for needing a resync and depending on other line conditions may have a detrimental effect. (One could say similar things in relation to ADSL).
Issues that you had with packet loss are rarely anything to do with your connection to the Cab and are usually related to problems over the network - either the BT part or the Plusnet end and dropping your PPP Internet session by logging into the Router and clicking Disconnect and waiting 30 seconds before clicking Connect will usually see your connection routed slightly differently often onto a different end point and a different Gateway.
As you found the problem went away, this strongly suggests the issue was directly related to the previous routing.
