Moving home

Hi all,
I'm moving home in a couple of weeks time, and I would like to move my plusnet connection with me.
Could anyone from support give me a hand? What do I need to do to arrange this?

    8am - 8pm Monday to Friday
    9am - 7pm Saturday
    9am - 6pm Sunday
