Broadband Troubleshooter

Hi
Trying to report a fault through the Troubleshooter but when I add any information to the 'additional information' box and click submit i get a 404 error
If i leave it blank I get the 'thanks' page.
I've tried various browsers but still the same
If i can't give the additional information then can i post my issue here?

The fault report has come through to us, I can add any additional information or you can do this directly at contactus.plus.net
