I finally got a response to the ticket I raised regarding the "SQLSTATE[42000]: Syntax error or access violation: 1142 INDEX command denied..." message. It reads:-
"Dear username,
I have referred this to one of our Database Analysts. Unfortunately our default SQL user construct does not grant you index privileges. You could create your index using the ALTER TABLE command. The code I believe for PhpGedView is open source and thus can be modified. Alternatively you can use our dedicated hosting service @ payh.co.uk [mysql5]."
I've added the indexes using the table ALTER command and everything seems to be running OK.
