Nameservers

Hi anyone who reads this:

I have a domain name that was originally hosted by freenetname, then madasafish but now i'm told it is hosted by plusnet.

The thing is, I want to transfer my domain to another hosting service.
I have paid for the transfer but have been told i must get my current provider to point the nameservers to my new hosts.

How can i do that please?
