NEWBIE CGI Question on Email

I know PN no longer support Front Page extensions so I am trying to get a comments page to work with CGI>
I am looking for a sample which would do even:
NAME: <name Field>
Email address <Email ID of sender>
Comments <Text box for comments>
This should be pretty simple but is stumping me
