CGi NAMES FOR phpbb SETUP

I'm being asked the following questions when setting up a new PHPBB3 board.
Can anyone help with the answers to the questions relating to database and server?

Database configuration

Database type:

Database server hostname or DSN:
DSN stands for Data Source Name and is relevant only for ODBC installs. On PostgreSQL, use localhost to connect to the local server via UNIX domain socket and 127.0.0.1 to connect via TCP. For SQLite, enter the full path to your database file.

Database server port:
Leave this blank unless you know the server operates on a non-standard port.

Database name:

Database username:

Database password:

Prefix for tables in database: [                                       ]


The prefix must start with a letter and must only contain letters, numbers and underscores.

The humbug and rumpus MySQL servers were retired after ccgi was moved to Hostopia hosting over 2 years ago. Databases on those servers were migrated along with website files. Configuration files for applications using the databases were updated to reflect the changed information.

If you login to the hosting control panel at hosting.plus.net and open the MySQL Manager application you will be shown a list of your migrated databases with hostname and username information. Relevant passwords can be obtained by looking in the configuration files for website applications which use these databases.
