Add-Ons menu icon only appears on certain pages

The Add-ons icon (and therefore the link to add/remove broadband add-ons) only appears on certain pages of the member centre's menus. It appears on the 'Billing' and 'Phone' pages, but is absent on all other pages.

There appear to be two versions of this menu - one including the 'Refer a friend' icon and one including the 'Add-ons' icon. The 'TV' page has its own special menu, which is a list of text links. Could perhaps these three different menus be united into a single menu with all icons present?
