I've not been able to ever reproduce any of these problems that you or drj have been having.
The only thing I can think of / see for you is I take it that's IE you've used there and you've got the Google Toolbar installed, try disabling it and see if you still get the problem. If you've got any other non-default Add-ons you could try disabling them as well.

Firefox.
I have removed google toolbar so will see what happens.

Didn't have to wait long
Has just done it again.
Interestingly, when Roundcube first started, it displayed the left hand pane showing inbox(21), this quickly changed to inbox(20) and then 20 inbox messages were listed. When 'prompted' to check again, Roundcube then correctly said inbox(21) and listed all 21 messages.

I've set my Mailbox View settings from the outset to Mark previewed messages as read after 30 seconds, but at the moment the preview pane is turned off.
Can you all see if you get the same behaviour as I'm seeing with whatever number of unread messages you have, I have currently got 73 (I've let it build up). I'm hoping it may show up some reproducible issue if you don't get the same.
With no preview pane, if I select a message and open it, the count in the Folder column for the Inbox goes down 1 immediately.
If I mark a message as read, the Inbox count goes down 1 immediately. If I mark a message as unread, the Inbox count goes up immediately.
If I delete an Unread message, the Inbox count goes down if I wait for the "Moving messages" ticker to stop, and also the Deleted Items count goes up.
If I delete an Unread message and don't wait for the ticker and select another message and open it, the Inbox count goes down once the message has opened - by 2 - the deleted message and the message being read, however the Deleted Items count does not go up until the next Folder column refresh.
If I move an Unread deleted item back to the Inbox, the Unread Inbox count goes back up immediately.

It happened today again on my work computer, a virtually standard installation of XPSP3, and Chrome.
My incoming emails are directed by my domain name provider to two places - Plusnet and Yahoo. This was originally set up when my former domain name provider names.co.uk started using an uncontrollable Spam filter meaning emails were going astray. The copy to Yahoo meant I could monitor this, which when demonstrated and reported, to no effect, meant I was forced to move my domain name provider elsewhere. Anyhow I've left the "mirroring" in place, and one consequence is that emails appear on my My Yahoo screen at work (in IE).
Today I noticed in the Yahoo listing that an important email had come in that I needed to reply to so I opened up RC in Chrome and it wasn't there! However I know by now that appearances can be deceptive so I hit "Inbox" and the missing email and several others then appeared.
So it's definitely happening in Firefox and Chrome, and on completely different systems.
There were about 40 emails in there at the time. I can't check this evening since having already read the email alert for this topic and come here, RC has logged itself out and is refusing to reconnect!

Just to clarify, you saw exactly the same as me with the Reads and Deletes etc.?

If I delete the Unread message, the Inbox count goes down after about one second (as the "Moving messages" ticker completes), however the Deleted Items count doesn't register since I've got the deleted folder set to flag all messages consigned there to read.
If I move the message back from Deleted to Inbox it remains as read and hence the counter doesn't go up.
However after trying some of these out several times I reached a point where the Inbox was thinking it had an unread message but didn't - see screenshot - when I hit Inbox a newly arrived email appeared (from another forum), so that's actually a new issue - I think previously the number displayed has matched the displayed number of messages albeit some were missing. Usually when the missing emails appear the inbox count goes up.
