Low Bandwidth

Connected 13 days ago at a starting speed of 12 Mbps, followed by a high number of disconnections over the first 10 days. Disconnections are now less frequent but download and upload speeds have both plummeted.

Do the estimates match with what is on dslchecker.bt.com?

That noise margin is very high (21.4db).
Looking at the connection logs I'm not surprised that the SNR has increased, your line has had a high number of disconnections.

Are you plugged into the master/test socket and is there any noise on the phone line?

Since the start, router has been connected directly to the ADSL faceplate master socket.
Removed faceplate and switched to using the test socket at ~21:00 12/08/15 (as seen on graph), but disconnections seem to have continued on 13/08/15 in spite of this (as also seen on graph).
Noise margin seems lower now, but seeing as the disconnections have still been occurring, it may be erratic:

Yes, there is substantial noise (tested with 2 different phone systems connected to the test socket).
