How to lodge a support call / complaint with "Online Ticket"

Need to contact support but all avenues are closed off.

1. Chat is offline
2. Phones not answered (1 disconnect after 1hr)

In the code of practice it mentions "online ticket" but I cant find any trace of this.
There is this page, https://portal.plus.net/wizard/index.html
but only accepts  specific criteria  

I just can get help anywhere; been trying off & on for weeks now!
Looks like Ill have to  write & post by letter.
