Downstream and now upstream speed problems

Hi there,
I've had problems with my internet speed for months now but its getting worse. Firstly, here are my router stats:

The downstream speed we used to get was around about 2.5mbps (and 3.5 when we first joined plus net years ago...funny how everyone else's connections are improving...), but now its stuck on this horribly low speed. I have tried the standard procedures and getting help from customer services but to no avail.
Also, tonight for some reason, even though we are getting our 448 upstream speed there seems to be something using up all its bandwidth (or it could be something completely different..) as when I ping any local server I get 200ms at best, so any online gaming just is not possible. I have a bandwidth monitor running which hardly shows any upstream usage and tried turning off all the other computers in the house. Called up customer support and everything seems fine there end, so haven't got a clue what the problem is.
Any help would be much appreciated - the internet wasn't too bad when it was JUST the downstream speed thats terrible, but now its both is absolutely awful. Let me know if you need any other details.

HI Doug,
I see our faults team have tested and asked you for three possible engineer appointment times. These have to be between either 8am-1pm or 1-6pm we cannot specify a time within these periods, only that the engineer will arrive between those times.
