Broadband speed falling again!

Please help.  Since July my broadband speed has been falling.  I first noticed buffering while attempting to watch catch-up TV so I did a check and here is the history that was provided by the plusnet speed test.

Today 16:20 2211 kbps (276kB/s) 361 kbps (45.1kB/s)
Sep 21, 15:54 2458 kbps (307kB/s) 373 kbps (46.6kB/s)
Aug 20, 17:01 3015 kbps (377kB/s) 378 kbps (47.3kB/s)
Jul 12, 16:03 3627 kbps (453kB/s) 321 kbps (40.1kB/s)
Jul 12, 13:58 3876 kbps (485kB/s) 339 kbps (42.4kB/s)
The last time that this happened is was because the line had been "banded".  Would someone please have a look and see if it has happened again (I think that it is the 3rd time in 12 months).  Thanks.

Hi TheProfessor,
I've run through a few tests on your line and it looks as though there is an issue with the phone service which is having an impact on the broadband speeds.BT have asked up to raise this to them however it looks as though they need to book an appointment in order to attend your address.
If you can drop me a PM with 3 dates and times for the appointment I should be able to get this booked for you. The appointments will run from Monday to Friday and will either be an AM (8am to 1pm) or PM (1pm to 6pm) time slot.
